چکیده
OBJECTIVE To determine the effects of prone positioning on cardiorespiratory stability and weaning outcome of preterm infants during weaning from mechanical ventilation. METHODS From January to December 1999, a sample of 42 preterm infants, with birthweight < 2,000 g, mechanically ventilated in the first week of life, were randomly divided, in the beginning of the weaning process, into two groups according to the position: supine position (n = 21) or prone position (n = 21). Heart rate, respiratory rate, transcutaneous oxygen saturation and ventilatory parameters were recorded every one hour. Length of the weaning process and complications were also assessed. RESULTS In both groups the mean gestational age was 29 weeks, most of the patients presented very low birthweight and respiratory distress syndrome. The mean length of the weaning process was 2 days. There were no differences between the groups regarding respiratory rate, heart rate and transcutaneous oxygen saturation, however, oxygen desaturation episodes were more frequent in supine position (p = 0.009). Ventilatory parameters decreased faster and reintubation was less frequent in the prone group (4% versus 33%). No adverse effects of prone positioning were observed. CONCLUSION These results suggest that prone position is a safe and beneficial procedure during weaning from mechanical ventilation and may contribute to weaning success in preterm infants.

Background and Objective: The results of previous studies show the neonate position is effective in improving oxygenation . Therefore, this study aimed to compare the prone and supine positions on oxygen saturation in preterm infants who were weaned from mechanical ventilation. Materials and Methods: This clinical trial carried out on 40 preterm infants who were weaned from mechan...
متن کاملThe impacts of prone position on the blood oxygen saturations and heart rates of preterm infants under the mechanical ventilation
Background: The methods of baby's position are an important critical factor in ventilation and oxygenation of tissues. Although the prone position as one of the recommended position has beneficial effect on the development of premature infants, little research has been done. Methods: In a clinical trial among mechanically ventilated infants, hospitalized in the neonatal intensive care units (N...

Background: Continuous positive airway pressure (CPAP) is used as respiratory support in preterm neonates; however, the best weaning method has not yet been determined. In this study, we compared sudden complete and gradual weaning from nasal CPAP (NCPAP) in preterm newborns. Methods: This randomized controlled trial was conducted on 62 prete...
متن کاملComparison of the effect of pressure support ventilation and volume assured pressure support ventilation on weaning patients off mechanical ventilation after cardiac surgery
Background: Weaning from mechanical ventilation in the intensive care unit is an important step in the treatment process. More patients in less than 6 hours after cardiac surgery can be separated from mechanical ventilation and extubated. However, 20-40% of patients after cardiac surgery, due to dysfunctional ventilator weaning response (DVWR), still remain under mechanical ventilation. Therefo...
متن کاملWeaning newborn infants from mechanical ventilation
Invasive mechanical ventilation is a life-saving procedure which is largely used in neonatal intensive care units, particularly in very premature newborn infants. However, this essential treatment may increase mortality and cause substantial morbidity, including lung or airway injuries, unplanned extubations, adverse hemodynamic effects, analgosedative dependency and severe infectious complicat...
